PLEASE READ THIS DOCUMENT CAREFULLY
You are being evaluated for participation in a medical weight loss program. This program may include one or more of the following:
– Physician-supervised nutritional and behavioral counseling
– Caloric intake recommendations
– Exercise guidelines
– Lifestyle modifications
– Prescription medications, including FDA-approved or compounded formulations
– Laboratory testing and ongoing medical assessments
Treatment is individualized, based on your medical history, clinical evaluation, goals, and response to interventions.
You may be prescribed one or more medications as part of your weight loss plan. These may include:
– FDA-approved anti-obesity medications
– Appetite suppressants or metabolic enhancers
– GLP-1 receptor agonists (e.g., Semaglutide, Tirzepatide)
– Lipotropic injections (e.g., MIC-B12)
– Other agents that influence metabolism, hunger, or satiety
– Compounded medications prescribed when deemed medically necessary
The use of prescription medications is at the discretion of the licensed provider and is based on your health profile, goals, and clinical eligibility.
Some treatments may involve compounded medications, which are custom-prepared by licensed U.S. pharmacies based on individual patient needs when FDA-approved alternatives are unavailable or unsuitable.
Please note:
– Compounded medications are not reviewed or approved by the FDA.
– These medications are prepared in accordance with U.S. pharmacy compounding laws.
– All partner pharmacies used by DrWell are licensed, regulated, and meet applicable state and federal compounding standards.
– DrWell is LegitScript certified, which means our platform and practices meet strict standards for legality, safety, and transparency in healthcare.
While many patients benefit from medical weight loss treatment, all interventions—whether behavioral, nutritional, or pharmacologic—carry risks. These may include, but are not limited to:
Lifestyle-Based Risks:
– Nutritional deficiencies
– Muscle loss
– Fatigue or dizziness from caloric restriction
– Emotional or psychological effects from weight loss attempts
Medication Risks:
– Nausea, vomiting, diarrhea, or constipation
– Headaches, fatigue, or injection site reactions
– Low blood sugar, especially if diabetic
– Gallbladder issues or pancreatitis (particularly with GLP-1s)
– Increased heart rate or blood pressure (with stimulant-based medications)
– Rare risks, including allergic reactions, hormonal effects, or mood changes
You will be monitored to assess for side effects or complications. Prompt reporting of symptoms to your provider is essential.
While many patients experience weight loss, no guarantee is made regarding the amount or duration of weight loss. Results vary depending on individual adherence, metabolism, health status, and other factors.
The success of the program depends significantly on your active participation, including commitment to dietary changes, physical activity, and follow-up care.
Treatment may be provided via telehealth, including:
– Secure video consultations
– Asynchronous provider reviews of submitted health information
– Digital prescription services
You understand and agree that:
– Telehealth is subject to limitations in technology and clinical evaluation.
– Your provider may not be physically located in your state, but is licensed to treat patients in the state where you are located.

Some medications may be used “off-label,” meaning for purposes not explicitly approved by the FDA. This is a legal and accepted medical practice when supported by clinical research and expert judgment.
All prescriptions, whether standard or compounded, will be issued only if clinically appropriate and following an evaluation by a licensed DrWell provider.
DrWell providers are licensed in all 50 U.S. states. However, certain weight loss medications—especially compounded formulations—may not be available in every state due to pharmacy licensing and regulatory restrictions.
All patients must undergo a medical consultation with a DrWell provider via:
– Virtual video visit

Medication access is dependent on the outcome of this consultation and on regulatory availability.
